The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) Roof of the house at 2820 Florence Road, Powder Springs, GA: Employees were exposed to fall hazards while improperly using a harness, carabineers and a positioning belt with a rope grab located at the wearer's waist. The employees were on a 10:12 pitched roof at a height of approximately 13 feet from the eave to the lower level.
