29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Employees installing a new composite roof with a 8/12 pitch and 16/12 pitch roof were not provided with fall protection, exposing them to an unprotected fall:  a)  On or about October 2, 2014, employees were observed working on the roof and top ridge edges exposed to approximately a 20 feet fall to the ground below, and  b)  On or about October 3, 2014, the employees were observed on the roof wearing harnesses that were not connected with lanyards and anchored to protect the employees from a fall hazard and exposed to approximately a 10 foot fall to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  On or about October 2, 2014, employees performing roofing activities accessed the roof area with extension ladders that the side rails did not extend at least 3 feet above the upper landing surface exposing them to the hazard of falling to the ground below.
