29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about October 01, 2014, the employer did not ensure employees engaged in decking activities were trained to recognize and avoid fall hazards and the procedures to be followed in order to minimize or avoid those hazards.

29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about October 6, 2014, the employer did not provide hard hats to employees performing decking activities who were working at lower levels and exposed to the hazard of being struck by falling decking supplies.   Cirilo Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 943740, Citation Number 1, Item number 1 and was affirmed as a final order on December 17, 2013, with a final abatement date of January 3, 2014, with respect to a workplace located at 1231 Fowler Street, Keller, TX 76248.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  On or about October 01, 2014,  employees who were performing decking activities on a residential home were not provided fall protection.  The employees were exposed to a fall hazard at a height of over 6 feet to the ground below.  Cirilo Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 943740, Citation Number 1, Item number 3 and was affirmed as a final order on December 17, 2013, with a final abatement date of January 3, 2014, with respect to a workplace located at 1231 Fowler Street, Keller, TX 76248.
