29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation: Jobsite: West side of Units 16-17. Pump jack scaffold poles were unsupported by mud sills and base plates thereby exposing subcontractor employees to fall and struck-by injury hazards.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: Jobsite: Units 20 and 21 Subcontractor employees were exposed to a fall hazard from stairways that lacked hand rails and / or stair rails. Instance A: A stairway with an open side lacked both a hand rail and a stair rail. Instance B: An enclosed stairway lacked a hand rail. Whitman Homes, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1052 (c)(1) which was contained in OSHA inspection number 972591, citation number 1, item number 1 and was affirmed as a final order on July 11, 2014, with respect to a workplace located at 900 Greendale Avenue in Needham, MA.
