29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about October 3, 2014, on the north side of the construction site, employees were powering hand tools with an extension cord that had the outer sheathing pulled away at the end plug straining the internal wiring and exposing the employees to electrical shock hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On or about October 3, 2014, on the north side of the construction site, employees installing exterior insulated finish systems (EIFS) on a commercial building were working from a work platform elevated by a Z-45125 Genie forklift without fall protection exposing them to an unprotected fall hazard of approximately 25 feet.
