29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  On or about 10/6/14, at the above address jobsite, the employer did not ensure the use of face protective equipment by the kettle operator while the employee was exposed to contacting hot asphalt during the roofing work performed at the jobsite.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or a personal fall arrest system:  Instances: a) On or about 10/6/14, at the above address jobsite, an employee was exposed to a fall hazard of approximately twelve feet (12ft.) while conducting roofing work at third floor on the south side of the jobsite.  b) On or about 10/6/14, at the above address jobsite, an employee was exposed to a fall hazard of approximately twenty feet (20ft.) while conducting roofing work at first floor on the west side of the jobsite.
