29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) Work bench on front side, new residential construction located at 648 Double Branches Lane, Dallas, GA 30132: Employee was exposed to severe laceration and amputation hazards while using a Makita (Model 5007F) 7-1/4-inch Circular Saw without a lower guard which was operational. A wood shim was placed between the upper guard and the lower guard to prevent the lower guard from retracting after a cut was made.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  (a) South side, new residential construction located at 648 Double Branches Lane, Dallas, GA 30132:  The employer allowed employees to work on and from a pump jack scaffold system with the supporting poles not resting on a firm foundation exposing employees to collapse/fall hazards of approximately 13-feet, 8-inches to the lower level.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  (a) South side, new residential construction located at 648 Double Branches Lane, Dallas, GA 30132: Employees were exposed to falls of approximately 13-feet, 8-inches while installing Hardie-Plank Lap Siding and cornice board from a pump jack scaffold platform which was not provided with a fall protection system. The employer failed to provide guardrails along all open sides or provide personal fall arrest systems.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:  (a) South side, new residential construction located at 648 Double Branches Lane, Dallas, GA 30132: The employer failed to secure the bottom of the pump jack scaffold support poles to prevent accidental movement or collapse of the system during use. The support poles were not plumb or square which compromised the strength of the scaffold system. Employees were exposed to fall hazards from potential collapse of the system as supporting bracing or equivalent was not used.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, new residential construction located at 648 Double Branches Lane, Dallas, GA 30132: Employees were exposed to fall hazards while installing Hardie-Shake Siding on a 3/12 pitched roof at eave heights of approximately 12-feet-6-inches with no fall protection system in place.
