29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a) Forever 21, 1156 Four Star Drive, Mount Joy, Pennsylvania - The employer did not have a competent person involved in the job site, on or about October 4, 2014, in that none of the three employees who worked on the site on October 4, 2014 inspected the job site for hazards and none had authority to correct hazards. As a result, one of the employees worked in a junction box with energized wiring and contacted electrical current, which resulted in injuries. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

As a result, an employee worked on a live circuit and received an electrical shock, on or about October 4, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
