29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices ("MUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA.      a) Axford Avenue - Oxford Road, from Hill Street heading NE to Warren Road, Oxford, NJ 07863: Advanced warning signs were not posted in accordance with the MUTCD:         #1) The signs were sitting on the ground leaning against existing post or mounted to job-made PVC pipes configured to support the signs.            #2) The signs were obstructed by residential debris and not mounted on break away post.     As such, the traffic was not controlled in accordance with MUTCD Part VI 6F.03 which states: Warning signs shall be mounted at a height at least 5 feet measured from the bottom of the sign to the near edge of pavement, shall be crashworthy and unobstructed.     Condition occurred on or about 09/30/14.

29 CFR 1926.201(a): Flagger activities did not conform to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD), 1988 Edition, Revision 3 or the Millennium Edition, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual of Uniform Traffic Control Devices, Millennium Edition, December 2000 FHWA (Qualifications for Flaggers):    a) Axford Avenue - Oxford Road, from Hill Street heading NE to Warren Road, Oxford, NJ 07863: Flagging Operation was not conducted in accordance with the MUTCD:        #1) An employee was conducting traffic / flagging operations within the right of way of a public street when the STOP paddle he was using was being supported by a near by cone and he was directing traffic with both hands.     As such traffic was not controlled in accordance with the 2000 MUTCD Part VI 6E.04 which states: To stop road users, the flagger shall face the road users and aim the STOP paddle toward road users in a stationary position with the arm extended horizontally away from the body. The free arm shall be held with the palm of the hand above the shoulder level toward approaching traffic.     Condition occurred on or about 09/30/14.
