V.M. Mamus Painting, LLC, 654 Millcreek Mall, Erie, PA 16565, on or about October 6, 2014 and times prior thereto: An employee on-site was performing spray painting tasks below roofing operations. Roofing operations included, but were not limited to, installation of TPO roofing and metal flashing work at the roof edge. The employees were not wearing head protection when exposed hazards including, but not limited to, falling equipment and supplies and fly debris, including, but not limited to metal and metal cutting tools.
