29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by competent persons (i.e., a person who is capable of identifying existing hazards of the job site).  On or about 29 September 2014, and at times prior thereto, the employer did not provide for frequent and periodic inspections of the jobsite located at 4800 Bull Mountain Cove, exposing the employees to fall hazards of as much as approximately 34 feet to the concrete basement below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 29 September 2014, and at times prior thereto, three employees were installing a home elevator within a private residence that required work within the elevator shaft in which fall protection was not utilized, exposing the employees to fall hazards of as much as approximately 34 feet to the concrete basement below.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  On or about 29 September 2014, and at times prior thereto, three employees were installing a home elevator within a private residence that required work within the elevator shaft at elevated heights in which the employees had not been trained in fall protection, exposing the employees to fall hazards of as much as approximately 34 feet to the concrete basement below.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  On or about 29 September 2014, and at times prior thereto, three employees were installing a home elevator within a private residence that required work within the elevator shaft in which an extension ladder was utilized to span the shaft opening at an angle greater than 4 to 1, exposing the employees to fall hazards of as much as approximately 34 feet to the concrete basement below.
