29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      On or about October 7, 2014, at the above addressed jobsite, employees were exposed to a fall of approximately 20 feet while performing sheathing work at a residential construction site without fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about October 7, 2014, at the above addressed jobsite, an employee was utilizing a aluminum extension ladder to access the second floor without extending the ladder at least 3 feet above the upper landing surface, exposing the employee to a fall hazard of approximately 10 feet.
