The Alberto Zelaya Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315350470, citation number 1, item number 1 and was affirmed as a final order on May 06, 2011 with respect to a workplace located at 4402 NW 112 Ct. FL, 33178.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about October 07, 2014, at the above addressed jobsite, an employee was utilizing a aluminum extension ladder to access the Peak of the roof without extending the ladder at least 3 feet above the upper landing surface, exposing the employee to a fall hazard of approximately 20 feet.
