29 CFR 1910.27(d)(3): The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails:  a) At the facility on the northwest side of the warehouse, on or about and at times prior to October 2, 2014, the employer did not ensure the side rails of a fixed industrial ladder extended three and a half feet above the landing, exposing employees to the hazards associated with death, broken bones, lacerations, sprains, and bruises.  b) At the facility on the southwest side of the cleaner, on or about and at times prior to October 2, 2014, the employer did not ensure the side rails of a fixed industrial ladder extended three and a half feet above the landing, exposing employees to the hazards associated with death, broken bones, lacerations, sprains, and bruises.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) At the facility on the northwest side of the warehouse, on October 2, 2014, the employer did not provide for a lockout tagout program for two employees who changed hydraulic hoses in the engine compartment of a Bobcat model 7753 in which the bucket was elevated and not properly locked out, exposing the employees to the hazards associated with death, broken bones, lacerations, and bruises.  b) At the facility on the northwest side of the warehouse, on or about and at times prior to October 3, 2014, the employer did not provide for a lockout tagout program for an employee who was tasked with lubricating an industrial type ventilation fan, exposing the employee to the hazards associated with amputations and lacerations.  c) At the facility on the southwest side of the cleaner, on or about and at times prior to October 3, 2014, the employer did not provide for a lockout tagout program for an employee who was tasked with lubricating an industrial type ventilation fan, exposing the employee to the hazards associated with amputations and lacerations.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals.
