On or about Oct 7, 2014, on the east side of the structure, the scaffold did not have adequate firm foundations, exposing employees to a 16 feet 7 inch fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a.   On or about Oct. 7 2014 on the east side of a structure, three employees were using a scaffold that did not have suitable means to access the scaffold platforms, exposing employees to a 16 feet 7 inch fall hazard.

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway:  a.   On or about Oct. 7, 2014 on the east side of a structure, three employees building a ladder well by installing cement blocks were using a scaffold that did not have guardrails, exposing employees to a 16 feet 7 inch fall hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arr  a.  On or about Oct. 7, 2014, on the north side of a flat surface, an employee installing cement blocks, while building an ladder well was not protected from an 11 feet fall hazard by the use of a personal fall protection system.

29 CFR 1926.602(c)(1)(vii): Unauthorized persons were allowed to ride on a powered industrial truck:  a.  On or about Oct. 7, 2014 an unauthorized employee was being driven on the forks of an all terrain forklift, exposing the employee to a fall and struck-by hazard.
