29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:               Or, in the alternative,     29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Employees performing roofing work on a roof 6 feet or more above ground are not protected from fall hazards by a fall protection system.     The employer fails to assure effective protection against falls as required by the standard, as demonstrated on September 20, 2014, when an employee was working on a residential roof at 120 Marcia Drive, Altamonte Springs, FL 32714 without fall protection and was exposed to falls of approximately 9 feet.     Collis Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 739143, Citation 01 Item 001 and was affirmed as a final order on May 23, 2013, with respect to workplace located at 256 Chandler, Oviedo, FL 32765.
