At facility- On or about October 07, 2014, employees were required to operate powered industrial vehicles. The employees did not receive the required evaluation on the safe operation of powered industrial vehicles within the past three years.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. a. The facilities yard area- On or about October 07, 2014, the employer did not perform an examination of the powered industrial equipment to check for defects, the right front headlight was inoperable, thereby exposing employees to serious struck by hazards.
