29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the rotating chuck:     a) Die Maintenance Shop: The employer failed to ensure that a Clausing 20 speed heavy duty drill press, model #22T4 serial #36F644T044H2, used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.    b) Die Maintenance Shop: The employer failed to ensure that a Vectrax heavy duty drill press, model #72526019 serial #1012J011, used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.   c) Die Maintenance Shop: The employer failed to ensure that a MCS heavy duty drill press used by the employees was equipped with a chuck guard.  Employees were exposed to laceration hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Die Maintenance Shop: the employer did not ensure that the work rest on the Sunex 8" bench grinder, model #5002A, was adjusted closely to the wheel. The opening was one and a half inches, exposing employees to severe lacerations and/or eye injuries.

The opening was two and a half inches, exposing employees to severe laceration and or eye injuries.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:   (a) Die Maintenance Shop: Employees had no ready/safe access to the 600 volts emergency power box blocked by the S&T Dynamics cylindrical grinding machine, model TGUA-32, exposing employees to 1st, 2nd, and 3rd degree burns and/or electrical shock.
