29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b). Worksite: The employees who were working from the second story of the residence at a height above six feet were not protected from falling by any means including an adequately installed guardrail system or by the covering of floor holes. On or about 10/2/2014 an employee sustained serious injuries after falling through a floor hole of a residence under construction located at 142 Warrington Round, Danbury, CT. Gamma Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 759541, citation number 02, item number 001, issued on 12/27/2012, with respect to a workplace located at 1208 Broad Street, Bridgeport, CT and became a final order on 1/9/2013. Gamma Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 328949, citation number 01, item number 01, issued on 5/7/2012, with respect to a workplace located at Reserve Road, Building 32 in Danbury, CT and became a final order on 5/25/2012.
