29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a- Toyota Autoland, Service Department: 1- Bays, including but not limited to #4, #9, #42 & #60 - The exhaust ventilation holes, approx. 4" x 4", which are stationed on the floor at each bay, were missing their covers. 2- Front side, waste water drain - A portion of the water drain grate, approx. 9" across x 4" wide, was missing. Conditions noted on or about 10/7/14.
