29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices. a) Distillation - On October 28, 2014 the employer had not developed nor required the use of administrative controls in accordance with API 520 Part II Paragraph 6.5 Sizing, Selection and Installation of Pressure -Relieving Devices which requires that administrative controls be in place that will prohibit the inappropriate closing of isolation valves in pressure relief system piping such as but not limited to valve 01 on the 6"-PG-SGP-309 line on feed column D-DC-C-006 drawing number ALB--D-F221, exposing employees to fire and explosion hazards.
