29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a. In the molding and bending department, employees were exposed to an amputation hazard, in that, no point of operation guarding was provided during the bending of metal parts on the AccurPress press brake Model # 72514, or the AccurPress Advantage, on or about 10/07/2014 b. In the de-burring and tubing departments, employees were exposed to an amputation hazard, in that, there was no cup guard on the Powermatic Drill Press # 155C/ 32 or the Rockwell Delta drill Press155B SN# 14G4517 which were used to drill holes into stainless steel sheets, on or about 10/07/2014. c. In the tubing department, employees were exposed to an amputation hazard, in that, while cutting metal tubing with the Thomas cold saw, no guard was present leaving 5 inches of the blade exposed during operation, on or about 10/07/2014 d. In the finishing department, employees were exposed to an amputation hazard, in that, while cutting 1 inch Styrofoam sheeting, 6 inches of the blade of the Rockwell SN #18114330 band saw was exposed, on or about 10/07/2014.
