29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 8, 2014, and times prior thereto, employees performing roofing activities were using an extension ladder that did not extend at least (3) feet above the upper landing surface of the roof for access and egress, exposing the employees to a fall hazard of up to (8) feet to the ground below.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about October 8, 2014, at a new residential construction site, employees who worked without fall protection while engaged in roofing activities were exposed to an unprotected fall hazard of approximately 9 feet to the ground.   Mr. Ismael Sierra was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 764841, citation number 1, item number 1 and was affirmed as a final order on April 30, 2013 with a final abatement date of May 10, 2013, with respect to a workplace located at 8600 Vatican Drive, McKinney, Texas.
