29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition:    (a) On October 08, 2014, at the facility located at 1903 Woodland Ave, Kansas City, MO., employees were exposed to serious injuries, in that one of the Safety Retaining Pins on the #107 Toyota Forklift was missing. The unit had been inspected the morning of this inspection by the operator and was still in operation. As a result, employees were exposed to serious injuries if a load was accidentally dropped by the forklift.               Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

One employee hired in July of 2014 was operating forklift equipment and had not completed the initial training as of the date of this inspection.            UNITED BEVERAGE COMPANY was previously cited for a violation of this Occupational Safety and Health standard - 29 CFR 1910. 178(L)(1)(i) which was contained in OSHA Inspection Number 314470840, Citation Number 01, Item Number 001, and was affirmed as a Final Order on January 25, 2013, with respect to the workplace located at 31903 Woodland Ave, Kansas City, MO. 64108.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
