In the spinner department, the 480 volt conduits running along the edge of spinners #10 and #11 were bent, broken and corroded.    b. In the End Plate Department on mold oven #1, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box.     c. In the End Plate Department on mold oven #2, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box.     d. In the Metal Storage Area, the power control box on/off switches were broken for compressor #2 and recessed in the control box requiring that the employees put their fingers into the control box.     e. In the Saw Department on Saw #1, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box.    f. In the Saw Department on Saw #2, the power control box on/off switches were broken and recessed in the control box requiring that the employees put their fingers into the control box.    g. In the Machine Shop on lathe #24, the power control handle was broken and missing.     h. In the Machine Shop on lathe #40, the power control handle was broken and missing.

In the Foundry Department next to furnace C & D, the floor fan's cord was run between a metal floor cover and the concrete floor exposing it to a pinch point.   b. In the AmStock Area, the floor fan's cord was run under a pallet and metal guard frame that was cut and damaged in several areas along the cord.   c. In the Shipping Department, the Dewalt circular saw's cord was cut and damaged in several areas along the cord.   d. In the Machine Shop, the Black & Decker HD 6 inch Grinders cord was damaged in several areas along the cord and had been repaired with electrical tape.   e. In the Heat Treat Department the job made cooler's cord was run through a hole in the wall exposing it to sharp edges of the sheet metal.

On or about October 08, 2014, and at times, prior thereto, the employer did not ensure cords and cables were connected to devices so that strain relief was provided, exposing employees to the hazards of electrical shock:    a. In the Saw Department, the TPI Industrial floor fan had the electrical power cord pulled from the fan casing.    b. In the Revert Area, the industrial fan's power cord had the outer sheathing pulled from the power control box.     c. In the CNC Department, on the #4 CNC Machine on the Chip Tote Conveyor's motor control, the outer sheathing of the wiring was pulled away from the motor.     d. In the CNC Department, on the #3 CNC Machine on the Chip Tote Conveyor's motor control, the outer sheathing of the wiring was pulled away from the controls.     e. In the CNC Department, on the #6 CNC Machine on the Chip Tote Conveyor's motor control, the outer sheathing of the wiring was pulled away from the controls.
