29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: location: intersection of corridors 101a and 101b on or about: 10/8/14 a) Employees were using electric cords and a portable GFCI to power equipment including, but not limited to a Ridgid 975 pipe threader. The cords and GFCI were missing their grounding pins.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: location: intersection of corridors 101a and 101b on or about: 10/8/14 a) Employees were using a portable GFCI to power equipment including, but not limited to a Ridgid 975 pipe threader. The strain relief of the GFCI was broken, and the outer plastic sheath was pulled away from the box.
