29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   (a)   PRODUCTION AREA NEAR FRONT ENTRANCE - Employees used an electrical receptacle box that was designed to be secured to a structure, as a homemade extension cord to provide 220 volt power to welding equipment, thereby exposing employees to electric shock hazards, on or about October 8, 2014.  (b)   MAIN WELD TABLE AREA - Employees used electrical receptacle boxes that were designed to be secured to a structure, as hanging pendant style power service drops, to provide power to power tools including grinders and welders, thereby exposing employees to electric shock hazards, on or about October 8, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  (a)  NEAR FRONT ENTRANCE OF SHOP - Employees used a homemade extension cord fabricated from flexible cable and an electrical receptacle box, and the strain relief where the cable entered the receptacle box had been compromised, thereby exposing employees to electrical hazards, on or about October 8, 2014.  No abatement certification or documentation required.
