Employees did not have trenching and excavation training. B.) Filer Plumbing, Heating & Construction, Inc., 182 George Junior Road, Grove City, PA 16127, on or about October 8, 2014 and times prior thereto: Employees were operating a gasoline powered chain saw to perform operations including, but not limited to, cutting down a tree. Employees did not have personal protective equipment training on the items to be worn while operating a chain saw. C.) Filer Plumbing, Heating & Construction, Inc., 182 George Junior Road, Grove City, PA 16127, on or about October 8, 2014 and times prior thereto: Employees were performing plumbing tasks that included, but was not limited to, and working in an excavation and cutting PVC pipe with a reciprocating saw. Employees did not have personal protective training that cover the use of items, including, but not limited to, eye protection and gloves. D.) Filer Plumbing, Heating & Construction, Inc., 182 George Junior Road, Grove City, PA 16127, on or about October 8, 2014 and times prior thereto: Multi-purpose dry chemical fire extinguishers were on-site and available for employee use. Employees did not have training on proper selection and use of portable fire extinguishers. E.) Filer Plumbing, Heating & Construction, Inc., 182 George Junior Road, Grove City, PA 16127, on or about October 8, 2014 and times prior thereto: Employees were operating an electric powered reciprocating saw to perform duties including, but not limited to, cutting PVC pipe. Employees were not provided with training on construction related electrical safety topics. F.) Filer Plumbing, Heating & Construction, Inc., 182 George Junior Road, Grove City, PA 16127, on or about October 8, 2014 and times prior thereto: Employees were operating an electric powered reciprocating saw to perform duties including, but not limited to, cutting PVC pipe.
