29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:     (a) House Construction Site: On or about October 8, 2014, and at times prior, employees were exposed to a fall of approximately ten feet to the ground below while installing asphalt shingles on a 6:12 pitch roof.    (b) Garage Construction Site: On or about October 8, 2014, and at times prior, employees were exposed to a fall of approximately ten feet to the ground below while installing asphalt shingles on a 4:12 pitch roof.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:    (a) Construction Site: On or about October 8, 2014, and at times prior, employees were exposed to a fall of up to approximately ten feet to the ground below using a ladder while using a ladder set at excessive angle.
