29 CFR 1910.147(c)(1):    Energy control program. The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative.    On 10/6/2014, Semco Plastic Company, Inc. had not established a program consisting of energy control procedures, employee training and periodic inspections.  The company did not have a written program consisting of energy control procedures, affected and other employees had not been trained and no periodic inspections had been conducted.

29 CFR 1910.212(a)(3)(ii):     The point of operation of machines whose operation exposes an employee to injury, was not guarded.     On 10/6/2014, employees from Semco Plastic Company, Inc. were exposed to point of operation hazards.  Several vertical band saws were not guarded due to the guards not being properly adjusted.  One employee was operating an improperly guarded Lincoln vertical band saw, serial # B30558, to cut excess material.

29 CFR 1910.215(b)(9):  Exposure adjustment.  Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section were exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch.  On 10/6/2014, an employee from Semco Plastic Company, Inc. was exposed to struck-by hazards.   The tongue guards were missing on a Pro-Tech pedestal grinder and Delta bench grinder in the maintenance area of the plant.
