29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Along the north end of roadway, on or about October 7, 2014, an employee was exposed to a cave-in inside a trench measuring approximately 9 feet in depth by 7 feet in width by 16 feet in length without any type of adequate cave-in protection while trying to locate a water main. The Damco Utility, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 315626291, citation number 1, item number 2 and was affirmed as a final order on 08/24/2011, with respect to a workplace located at A St. E, Universal City, TX 78150. The Damco Utility, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 314303462, citation number 1, item number 2 and was affirmed as a final order on 12/16/2010, with respect to a workplace located at 12411 Wetmore Rd., San Antonio, TX 78247.
