29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: JOB SITE: 1385 Industrial Drive, Kahoka, MO 63445, on or about 10/7/2014, at this facility, safety checks on powered industrial trucks revealed the following defects and were not taken out of service: 1) CAT Propane Forklift Unit# CN1552: missing propane tank mounting bracket, emergency brakes not working properly, strobe light missing, and driver side rear tire need replacement. 2) YALE Propane Forklift S/N A875B__299Y: Right rear brake light & horn not working and motor fan belt was cracked. 3) JGB 940 Lift: Rear brakes and driver side rear turn signal light not working and driver door (missing a window on lower panel). GREGORY CONTAINER COMPANY was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(1), which was contained in OSHA inspection number 974571, citation number 1, item number 7, issued on 6/02/2014, and was affirmed as a final order on 6/27/2014, with respect to a workplace located 1385 Industrial Drive, Kahoka, MO 63445.
