29 CFR 1917.50(b)(1): The certification required was not performed in accordance with 29 CFR Part 1919 by persons then currently accredited by the Occupational Safety and Health Administration (OSHA): Rig Tenders Dock: On or about October 8, 2014 and at times prior thereto, certification of the Sany SCC3000US, equipment number 181-002, Lattice Boom Crane had not been certified for use as a marine cargo handling device. Certification by a person accredited by OSHA ensures the crane is capable of operating in the maritime environment.

29 CFR 1917.111(b): Maximum safe load limits, in pounds per square foot (kilograms per square meter), of floors elevated above ground level, and pier structures over the water were not conspicuously posted in all cargo areas: Warehouse: On or about October 8, 2014 and at times prior thereto, the mezzanine storage above the office spaces was not posted with its maximum safe load limit. This condition exposed the facility to overloading of the structure.
