29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) worksite - An employee was working directly below ongoing concrete form installation without head protection; on or about 10/7/2014.  Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:  a) worksite - Employees installing concrete forms approximately 12 feet above the ground did not use fall protection; on or about 10/7/14.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
