29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: a) Clean Up Department - Employees used a Cobra DLX upholstery cleaning machine where the outer insulation on the electrical power cord had been pulled back from the plug exposing the inner insulated wires, observed on or about 10-7-14. Abatement certification required.
