Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were injured when the ceiling they were standing on collapsed causing them to come in contact with the concrete floor below.        (a)     Prost Brewing Company at 2540 19th St., Denver, CO 80211: On and before October 2, 2014, employees above the Brewing Area, standing on a steel storage rack at an elevation of approximately 10-15 feet above the floor, were not adequately protected from falls in that the anchor connectors used in the Personal Fall Arrest System were standard hardware store-bought eyebolts that were not rated or load tested to ensure they were capable of withstanding (without breaking) a 5,000-pound (22.2kN) load.

29 CFR 1910.23(a)(2):  Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    (a)         Prost Brewing Company at 2540 19th St., Denver, CO 80211: On and before October 2, 2014, the employer did not ensure that a ladderway opening to the loft area was guarded by a swinging gate or so offset that a person could not walk directly into the opening. This condition exposed employees to a fall hazard of approximately 10 feet, 8 inches.

29 CFR 1910.303(g)(1)(i):  Workspace was less than 3 feet in front of electric equipment operating at 600 volts nominal, or less:     (a)       Prost Brewing Company at 2540 19th St., Denver, CO 80211: On and before October 2, 2014, the employer did not maintain workspace of at least 3 feet in front of electrical panels located at the back wall of the brew room in that access to the panels was restricted with only 19 inches of clearance in front. This condition exposed employees to electrical and mechanical hazards.
