29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.  The employer does not ensure that each platform on all working levels of scaffolds was fully planked or decked between the front uprights and the guardrail supports.  This violation was observed on or about October 8, 2014 when employees were exposed to a 13 foot fall hazard while doing masonry work form the second level of a scaffold without it being fully planked.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all working levels of scaffolds are fully planked or decked between the front uprights and the guardrail supports.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  The employer does not protect employees from falling when working on scaffold more than 10 feet above a lower level.  This violation was observed on or about October 8, 2014, at the job site where employees were exposed to a fall hazard when working from a platform of frame scaffold at approximately 13 feet above the ground without being protected from falling to the ground.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees working from a scaffold 10 feet or more above a lower level are protected from falling.

This violation was observed on or about October 8, 2014 at the job site where employees were exposed to an approximate 13 foot fall hazard when working on an frame scaffold without base plates and mud sills.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffold legs bear on base plates and mud sills.

This violation was observed on or about October 8, 2014 at the job site where employees were exposed to a fall hazard when working on an frame scaffold without supported scaffold poles, legs, post, frames, and/or uprights plumb and braced to prevent swaying and displacement. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    The employer does not ensure that employees have a ladder, stairway, or ramp to access a scaffold.  This violation occurred on or about October 8, 2014 when employees were exposed to an approximate 13 foot fall hazard while installing brick and rock siding on a two story residential structure from an elevated scaffold without adequate access.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have proper access to the scaffold platform.
