29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation. The employer does not ensure that scaffold legs bear on base plates and mud sills other adequate firm foundation. This violation was observed on or about October 8, 2014 at the job site where an employee was exposed to a fall hazard when working on an pump jack scaffold without base plates and mud sills. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffold legs bear on base plates and mud sills.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. The employer does not secure the scaffold poles to the structure at the bottom, top, and other points as necessary. This violation was observed on or about October 8, 2014, at the job site where an employee was exposed to a fall hazard when working on an elevated pump jack scaffold without the scaffold being secured at the bottom, top, and other points as necessary. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure scaffold poles are secured to the structure by rigid triangular bracing at the bottom, top, and other points as necessary.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. The employer does not ensure that employees have a ladder, stairway, or ramp to access a scaffold. This violation occurred on or about October 8, 2014 when an employee was exposed to a fall hazard while installing siding, cornices and windows on a residential structure from an elevated scaffold without adequate access. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have proper access to the scaffold platform.
