29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 8 October 2014, and at times prior thereto, a worker was using a pneumatic nail gun to perform framing activities without the use of eye protection, exposing him to the hazard of being struck by flying objects.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On or about 8 October 2014, and at times prior thereto, a 4-plug extension outlet was being used while having strain relief damage, exposing all 4 workers using it to an electrical hazard of approximately 120 volts.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 8 October 2014, and at times prior thereto, two workers were performing framing activities at the edge of the second story top plate without fall protection, exposing the workers to a fall hazard of approximately 23 feet to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.  On or about 8 October 2014, and at times prior thereto, two workers were utilizing 6 foot fiberglass stepladders to access the second story top plate by using the top step or second to top step, exposing the workers to a fall hazard of approximately 10 feet 8 inches to the floor below.
