29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about 8 October 2014, and at times prior thereto, three workers were using pneumatic nail guns to perform roofing activities without the use of eye protection, exposing them to the hazard of being struck by flying objects.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   On or about 8 October 2014, and at times prior thereto, three workers were performing roofing activities which required equipment and materials to be moved around above on the roof while any of the three workers could come down off the roof and be underneath the roof edge, exposing those workers to the hazard of being struck by falling tools, materials or debris.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems.  On or about 8 October 2014, and at times prior thereto, three workers were performing roofing activities on a 10/12 and 6/12 sloped roof without fall protection, exposing the workers to a fall hazard of approximately 13 feet 8 inches to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.   On or about 8 October 2014, and at times prior thereto, three workers were using a 24 foot portable aluminum extension ladder to access the roof of a residence under construction in which the ladder side rails were not extended at least 3 feet above the roof landing surface, exposing the workers to the hazard of falling approximately 13 feet 8 inches to the ground below.
