29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 9 October 2014, and at times prior thereto, four workers were cutting pieces of Hardie Plank siding and handing them up to workers above who were performing siding installation activities, exposing the workers to the hazard of being struck by falling materials, tools, and debris.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about 9 October 2014, and at times prior thereto, a worker was elevated and operating a Genie telescoping boom lift without using a body belt with lanyard, exposing the worker to a fall hazard.
