29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about 9 October 2014, and at times prior thereto, the employer did not enforce safety programs for the frequent and regular inspections of the job site to prevent hazardous conditions, exposing workers to the following hazards:  1) Two siding installers were cutting Hardie Plank siding without using a wet method and/or wearing respirators, exposing the workers to the hazard of inhaling silica dust. 2) Eleven siding installers were working without hard hats while handing pieces of siding above to other employees who were performing installation of the siding over an approximate 2 month period, exposing the workers to the hazard of being struck by falling materials, tools, and debris. 3) Two siding installers were cutting Hardie Plank siding with both a hand-held circular saw and a jig saw without the use of eye protection, exposing the workers to the hazard of being struck by flying objects. 4) Four siding installers were operating a Genie telescoping boom lift without using a body belt with lanyard, exposing the workers to a fall hazard. 5) A siding installer was operating a Genie telescoping boom lift without being trained on the lift, exposing the worker and those working nearby to the hazard of being struck by the vehicle.
