29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:      On or about October 29, 2014 at the above addressed job site, an employee working in the shop operating an automated bending machine and in proximity to a steel cutting machine was exposed to noise dose of 69.9% or an equivalent of 87.4 dBA which is capable of causing permanent hearing loss.  Monitoring was performed for 411 minute sampling time and zero exposure for the remaining 69 minutes of the 8 hour shift.
