Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about October 10, 2014, on the south side of commercial building, employees engaged in roofing activities without fall protection were exposed to an unprotected fall hazard of approximately 16 feet to the lower level.
