On or about October 4, 2014, at the south side of the building, one employee using a pneumatic air nail gun to install plywood on a residential roof was not wearing eye protection, exposing himself to being struck by flying debris or nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a. On or about October 4, 2014, at the west side of the building, an employee installing fascia while standing on the leading edge top plate was not protected from a 22ft-7in fall hazard by the use of a fall protection system. b. On or about October 4, 2014, at the south side of the building, an employee doing framing work while standing on the leading edge top plate was not protected from a 22ft-7in fall hazard by the use of a fall protection system. c. On or about October 4, 2014, at the south side of the building, an employee doing sheathing work on a 4:12 pitch roof was not protected from an 11 feet fall hazard by the use of a fall protection system. d. On or about October 4, 2014, at the north side of the building, an employee doing sheathing work on a 4:12 pitch roof was not protected from a 10 feet fall hazard by the use of a fall protection system. e. On or about October 4, 2014, at the north side of the building, an employee doing framing work while standing on the leading edge top plate was not protected from a 20 feet fall hazard by the use of a fall protection system.
