29 CFR 1910.67(c)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) 195 Wickaboag Valley Rd., West Brookfield, MA: On or about October 10, 2014, an employee was observed working aloft from a vehicle mounted aerial lift without being attached to the boom or basket. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.333(c)(3): Work was preformed near overhead lines, and the lines were not deenergized and grounded, nor were other protective measures provided before work was started: (a) 195 Wickaboag Valley Rd., West Brookfield, MA: On or about October 10, 2014, an employee was working from a vehicle mounted aerial lift in the electrical right of way space to trim trees without the lines being deenergized nor were any other protective measures employed before starting work in the electrical space. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
