29 CFR 1910.179(i): Crane(s) equipped with a power traveling mechanism were not provided with a gong or other effective warning signal:    On or about October 8, 2014, and at times prior to, at the RBI area in fabrication department an overhead crane operated by a remote control device to lift heavy steel parts was not provided with the means (audible alarm or strobe light) to warn employees working in the areas when the cranes were on the move.  Employees were exposed to struck-by hazards.

Employees were exposed to flying fragments in the event of an exploding wheel.

Employees were exposed to flying fragments in the event of an exploding wheel.

This would cause the wheel to be out of balance and having greater chance to shatter.
