29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided to prevent pull from being directly transmitted to joints or terminal screws:   On or about October 8 2014 at the jobsite employees were exposed to electrical shock hazards when the employer failed to ensure a flexible cord used by employees had the proper strain relief.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees and/or materials without tipping, or had guardrails which block employee access to the cantilevered end.    On or about October 8, 2014 at the jobsite employees were not prevented from working from a scaffold that had planking extended over 12 inches from the frame of the scaffold, exposing employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:   On or about October 8, 2014 at the jobsite an employee was observed using the cross braces of a scaffold to access the upper level.   San Antonio Lath and Plaster was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315172445 citation number 1 item number 1 and was affirmed as a final order on March 21, 2011 with respect to a workplace located at 1000 N. First St. Robstown, TX.
