29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: On or about October 8, 2014, at the jobsite, the black extension cord supplying power to a Hitachi drill, was severly cut and frayed, exposing employees to electrical hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: On or about October 8, 2014, employees who were installing metal support rails and siding on an overhead canopy at a new Valero gas station were not protected from falling, exposing the employees to an unprotected fall hazard of approximately 20 feet to the ground below.
