29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  On or about October 07, 2014, employees performing framing activities used frayed/cut flexible extension cords with the outer protective sheathing pulled away exposing the inner wire conductors to power electrical hand tools.  The employees were exposed to electric shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about October 07, 2014, employees performing framing activities on a residential framed structure were exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.
